1. STAGE 1: Claims

a. Contact People to see if they need medical supplies, including

(Compression Stockings, Medical Braces, Custom Made Orthotics, Orthopedic shoes, TENZ Machines)

b. Tell them to must come with prescriptions, like a family doctors note. (Extra 1.)

c. Make appointment about when they can come in with doctor, once the note is ready.

d. if they need any products, like shoes or stockings etc, email them the catalogs so they can choose before hand.

2. STAGE 2: When Patients Come With Doctor's note

1. Patients'job

a. fill out covid form

b. show ID and remove facemask to prove covid form its ID is true.

c. Fill Out Patient Intake Form

d. provide doctor’s note

e. pay the full amount or deposit of $100, and get recipt for future references

f. get measured by specialist for orthotics

g. make appoint for shoes pickup. might need dispense.

2. Specialist'job

a. come in and measure the patient, to fill out patient assessment form. (1.e)

b. generate lab order recipt

c. generate lab report assessment document.

3. Office Job

a. Take Measurement For patients if needed

b. fill out sticky note form (client register form)

c. then collect Covid Form (1.a), patient intake form (1.c), doctor’s note (1.d), recipt (1.e), specialist’s patient assessment form(2.a), and sticky note form (3.b) to main office.

d. arrange patient time to come in to pickup product

Stage 3. File Claims / Estimations for insurance company

Four files are needed when sending to insurance

1. doctor’s note

2. patient’s assessment from specialist

3. lab report

4.Recipt of payment

b. pay for rest of the payment if they have not paid in full

c. get product

d. sign product receipt.

a. get receipt for full payment. (4.b)

b. get a product receipt.
